Pay Range: $19 - $22 per hour

What You'll Do:

• Assist in the development and implementation of care plans that address residents' social, emotional, and psychological needs
• Support residents and families with discharge planning, community resources, and emotional support
• Advocate for residents' rights and work to resolve any concerns or issues
• Maintain accurate documentation and reports in compliance with state and federal regulations
• Collaborate with the interdisciplinary team to provide a holistic approach to resident care
• Help organize resident-centered activities and programs that promote social engagement and well-being

What We're Looking For:

• Experience in social services or healthcare preferred, but we are willing to train the right candidate

• Knowledge of community resources and basic computer skills a plus
