JFSA of Cleveland is actively seeking a part-time Bilingual ( English and Russian and/or Ukrainian) Registered Nurse (or LPN) Case Manager The Bilingual RN (or LPN) Case Manager provides comprehensive, professional, and compassionate nursing care to patients in their homes; assesses and treats clients according to the individualized plan of care; monitors and documents client condition, progress, and outcomes; provides nursing education to clients and their families. The Case Manager acts in accordance with State nursing laws and Agency policies. This position will provide a wide array of community-based services to adults diagnosed with serious and persistent mental illness to maintain community stability and improve quality of life. Therapeutic Behavior General Duties and Responsibilities: Responsible for completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and documenting significant changes in condition according to Agency policy and defined time frames. Thoroughly completes detailed clinical notes at each visit within required time frames; completing a physical assessment of the client, medication review, skilled interventions and teachings, and an assessment of the home environment. Under the direction of the RN Case Manager, completes nursing visits and provides clinical services as determined and authorized by the client's plan of care. Supervises all patients plans of care according to physician orders and within the required time frames. Responsible for the timely completion of clinical documentation, communicating status changes and care plan progress to all corresponding disciplines. Responsible for completing physical assessments within the time frames dictated by the regulating bodies. Assists with back-office-related tasks as needed. Provides clinical education and resources to clients and families within the community. Travels throughout the geographic market to provide nursing services and to supervise and evaluate homemaker/PCA staff. Participates in case conferences with the program director, as needed. Assists with Quality Improvement activities. Assists in providing accurate and timely billing and payroll information to the fiscal staff. Provides on-call coverage evenings, holidays, and weekends as needed in rotation with other supervisory nurses. Maintains confidentiality and adheres to HIPAA regulations on all clients. Attends in-services as scheduled. Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices. Also contributes to the control of hazards in specified areas of responsibility.Perform other duties or special projects are required or as assigned. Licenses/Certifications: Valid RN license in Ohio and or LPN Certification Current Ohio Drivers license and automobile insurance. Current TB test or chest x-ray.
