Job Duties:

• Provide and document quality nursing care to residents
• Administer medications and treatments as prescribed
• Monitor and report any changes in residents' conditions
• Collaborate with RNs and healthcare team members to implement care plans
• Maintain accurate medical records and charting
• Assist in admissions, transfers, and discharges
• Educate residents and families on care plans and wellness practices
• Ensure compliance with all policies and regulatory standards
• Respond to emergencies promptly and effectively

Qualifications:

• Current LPN license
• Strong clinical and organizational skills
• Compassionate and caring demeanor
• Effective communication and teamwork abilities
